导致老年性耳聋的因素很多,包括内在和外在因素。老年性耳聋包括以下若干原因:

一、抗氧化剂抑制活性氧:
活性氧(自由基)涉及广泛的健康问题,包括听力损失的发展和进展。氧化应激增加已在不明原因的突发性感音神经性听力损失患者中得到证实,氧化应激会导致耳蜗细胞死亡1,2。抗氧化剂是一种具有中和破坏性氧化应激能力的化合物,是预防和治疗听力损失的一种有前途的治疗策略3,4。事实上,氧化应激抑制剂已被广泛研究用于预防和治疗由药物引起的耳朵损伤(耳毒性)引起的听力损失1。临床前证据表明,抗氧化剂最重要的作用可能是预防听力损失3。
1.N-乙酰半胱氨酸(NAC):
NAC可增加体内最重要的抗氧化剂之一谷胱甘肽的生成5。 NAC也是一种直接的自由基清除剂,这可能有助于其在预防和治疗听力损失方面的潜在作用6,7。
一项针对566名军事人员的随机安慰剂对照试验发现,NAC可以防止武器训练期间噪音引起的一些听力变化,尽管效果在统计上并不显著8。另一项随机对照试验在48名经常暴露在大噪音中的纺织工人中进行,发现每天1200mg NAC,持续14天,可减少与噪音相关的暂时性听力损失9。在一项针对41名突发性感音神经性听力损失患者的对照试验中,那些接受NAC和鼓膜内(通过鼓膜)注射类固醇地塞米松14天的患者获得了听到特定高音调(8,000 Hz)的能力,并且比那些单独接受鼓膜内类固醇注射的患者更有可能恢复听力10。通过219例突发性感音神经性听力损失患者的观察数据表明,在标准的口服皮质类固醇治疗中加入NAC,可能会改善听力恢复的某些方面11。
2.硫辛酸:
因其在预防和治疗听力损失方面的可能作用而受到研究关注,并在与年龄相关、噪声诱导和耳毒性相关的听力损失的临床前模型中显示出了前景12-15。α-硫辛酸的临床试验也显示出了希望。一项包括30名听力正常的健康志愿者的对照试验发现,在实验性噪声暴露前10天,每天服用600mg硫辛酸,比在噪声暴露前一小时服用600mg硫辛酸或不服用硫辛酸更好地减少了暂时的噪声引起的听力损失16。在一项针对46名年龄相关听力损失的老年受试者的临床试验中,每天服用60mg硫辛酸、600mg维生素C和300mg瑞巴米肽(Rebamipide,一种药物抗氧化剂)治疗至少八周,在所有测试频率下都能显著改善听力17。
3.维生素抗氧化剂:
更多地摄入维生素A、维生素C和维生素E似乎与老年性听力损失风险降低有关18。这种影响可能部分通过保护毛细胞免受氧化损伤来介导,正如一项大鼠研究所示,补充维生素A、C和E以及镁有助于减轻噪音性听力损失后的毛细胞死亡19。
在一项针对近3000名50岁以上成年人的观察性研究中,发现维生素A和E的饮食摄入量越高,听力损失的发生率就越低20。在一项包括2592名美国成年人数据的观察性研究中,β-胡萝卜素、维生素C、维生素E以及镁的摄入量都与降低听力损失的风险有关。该研究还发现,β-胡萝卜素、维生素C和维生素E作为一组与听力保护的关系比单个营养素与听力保护更密切21。一项系统性综述包括三项临床试验,共有279名参与者,发现将抗氧化治疗与维生素A、C和E结合治疗突发性感音神经性听力损失可以提高听力增益22。在一项对照临床试验中,70名不明原因的突发性感音神经性听力损失患者接受了7800mcg RAE(26,000 IU)β-胡萝卜素(维生素A原)、200mg维生素C、90mg(200IU)维生素E(d-α-生育酚)和50mcg的矿物质硒,每天二次,作为药物治疗和高压氧的辅助治疗,而56名类似的患者仅接受药物和高压氧治疗,持续30天。两组患者的听力都有所改善,但接受抗氧化剂补充剂治疗的患者听力改善幅度更大23。
4.辅酶Q10:
辅酶Q10(CoQ10)是一种抗氧化剂,在细胞代谢和能量生产中起着至关重要的作用。许多动物研究表明,辅酶Q10可以降低听觉系统组织中的氧化应激,并防止因衰老、噪音暴露和耳毒性药物而导致的听力损失24-27。在一项针对18名志愿者的非对照试验中,用专利水溶性辅酶Q10治疗30天缩短了实验性噪声暴露后听力恢复的时间28。一项包括60名年龄相关性听力损失患者的安慰剂对照试验发现,与每天服用50mg维生素E或安慰剂相比,每天服用160mg水溶性辅酶Q10治疗30天,听力在中等和较高频率上有所提高29。
一项观察性研究发现,30名不明原因的突发性感音神经性听力损失患者的CoQ10水平低于60名听力健康的类似受试者30。在一项试验中,120名突发性感神经性听力丧失患者单独或联合使用系统性类固醇治疗,两种治疗方法都能改善听力,但类固醇加辅酶Q10能更好地改善言语辨别能力31。
5.锌:
锌一种参与许多生理过程(包括神经系统功能)的矿物质,在内耳的耳蜗中大量存在。它是人体关键内源性抗氧化酶之一的成分,因此有助于保护脆弱的内耳组织免受氧化损伤32。一项随机对照试验招募了66名突发性感音神经性听力损失的参与者,将皮质类固醇治疗加上每天78mg口服葡萄糖酸锌(提供20mg锌)与单独使用皮质类固醇进行了比较。锌组的听力增加更大,恢复的可能性更高,听力改善的程度与血锌水平的增加量相关33。
6.甲硫氨酸:
蛋氨酸(即甲硫氨酸)是产生谷胱甘肽所需的氨基酸。在一项小型、探索性、随机、安慰剂对照试验中,27名癌症患者正在接受已知会导致耳毒性的化疗药物治疗,那些口服D-甲硫氨酸(100mg/公斤体重)和化疗的患者可以免受药物诱导的听力损失34。对动物的研究也表明,补充D-甲硫酸钠可能有助于预防噪音诱导的听力下降,尤其是在暴露前或暴露后36-72小时内服用,并且可预防药物相关的耳毒性35-38。
二、其他有益营养:
1.镁:
镁摄入不足以及血液和细胞镁水平不理想是常见的39,40。这个问题与心血管疾病、糖尿病、高血压和其他慢性疾病的高发病率有关。镁在炎症和氧化过程中也发挥着重要作用41,42。镁可以通过多种机制预防听力损失,包括其抵消过量钙浓度的作用,通过促进血管舒张改善血液流动的能力,以及发挥抗氧化作用以帮助调节兴奋性神经传递的功能43。
在1993年发表的一项双盲研究中,一组320名军事人员在两个月的大部分时间里服用了约160mg的元素镁(以天冬氨酸镁的形式)或安慰剂。试验结束时,安慰剂组永久性听力损失的发生率是镁组的2倍44。在同一研究组的一项类似研究中,300名年轻健康的新兵在枪械训练中接受了相同的镁制剂。这项研究发现,补充镁会增加红细胞镁的浓度,听力损失的一些指标与红细胞镁状态呈负相关。换言之,在红细胞镁含量较高的参与者中,听力损失不太普遍45。一项在以色列进行并于2002年发表的对照临床试验招募了133名不明原因的突发性听力损失参与者。研究受试者接受了静脉注射镁和吸入碳合气(Carbogen,即5%CO2和95%O2的气体混合物,吸入时可引起血管舒张)治疗或单独吸入碳合气的联合治疗。碳合气可以通过增加内耳的血流量来帮助突发性听力损失。在这项研究中,静脉注射镁导致更高的听力恢复率46。
在一项随机对照试验中,51名耳蜗植入者每天口服315mg镁加18mg β-胡萝卜素、500mg维生素C和267mg维生素E(dl-α-生育酚)或安慰剂,持续106天。研究发现,那些接受镁加抗氧化剂治疗的人在三个月内比那些接受安慰剂治疗的人有更好的听力保护。在接受抗氧化剂和镁治疗的人中,这一差异使听力损失减少了8分贝47。一项对约2600名年龄在20-69岁之间的参与者的观察性研究发现,镁、β-胡萝卜素、维生素C和维生素E的摄入量越高,每一项都与更好的听力和高频有关21。
2.银杏叶:
银杏叶标准化提取物已被广泛研究其对认知障碍和痴呆等年龄相关疾病的可能益处48-50。在突发性感音神经性听力损失患者中进行了大量对照临床试验。一项对11项随机对照试验(共1069名参与者)结果的荟萃分析发现,银杏提取物与皮质类固醇疗法联合作为突发性感音神经性听力损失患者的初始治疗可能是有益的,但还需要更多高质量的研究来最终确定银杏对这种疾病患者的影响51。
一项观察性研究回顾了56名单侧突发性感音神经性听力损失患者的结果,其中30人接受了银杏提取物作为治疗的一部分。对于那些有严重听力损失的患者,在常规静脉注射皮质类固醇治疗的基础上加入银杏,其听力恢复程度明显高于单独使用皮质类固醇治疗52。
这些益处的可能机制包括银杏通过刺激抗氧化酶活性、减少反应性自由基和减少脂质过氧化来对抗氧化应激的公认能力53,以及可能防止噪音引起的耳蜗突触损伤54。
3.维生素B族和同型半胱氨酸:
叶酸和维生素B12是全身细胞(包括神经细胞)正常功能所必需的。这些维生素还有助于降低同型半胱氨酸的水平,当B12、叶酸和其他营养水平较低时,同型半胱氨酸是一种潜在的有毒化合物,可在体内积聚。同型半胱氨酸水平升高(高血同)与心血管疾病、中风和痴呆的风险增加有关,以及与心脏病相关的死亡55,56。血管功能障碍是同型半胱氨酸水平升高的重要病理后果,也是心血管疾病的已知独立因素57,58。一些数据表明,血管功能障碍和其他对血管健康的负面影响也在感音神经性听力损失中起作用2,59,60。
在一项针对20名20-30岁健康志愿者的随机安慰剂对照试验中,每天注射1mg维生素B12,持续7天,然后在第8天注射5mg,可以预防实验性噪声引起的听力损失61。在一项涉及近3000名50岁以上成年人的观察性研究中,血清同型半胱氨酸水平极高(>20mmoles /L)的个体的听力损失发生率增加了64%,血清叶酸水平较低的个体的发病率增加了37%62。另一项观察性研究同样发现,叶酸水平低与年龄相关的听力损失相关,而在动物模型中,叶酸缺乏被发现会导致耳蜗氧化应激和同型半胱氨酸积累导致过早听力损失63。其他临床前和观察数据表明,高血同与听力损失和耳蜗功能障碍有关64,65。
4.维生素D:
多项观察性研究发现维生素D缺乏与听力损失之间存在关联。一项针对1100多名70岁以上美国人的观察性研究发现,维生素D缺乏(<20ng/mL)与听力损失有关66。另一项针对3489名50岁以上人群的此类研究发现,维生素D缺乏通常与听力损伤相关,尤其与感音神经性听力损失相关67。在分析2型糖尿病患者数据的观察性研究中,低维生素D水平与更高的听力损失风险相关68,69。在另一项观察性研究中,即使没有听力损失,充足的血清维生素D似乎也能保护耳蜗健康70。
5.欧米伽3脂肪酸:
一项使用护士健康研究II中65,215名女性数据的研究发现,在大约18年的监测过程中,鱼类摄入的ω-3脂肪酸越多,患听力损失的风险越低。事实上,与每月吃鱼少于一次的人相比,每周吃二到四次鱼的人患听力损失的可能性降低了20%71。
另一项研究调查了2956名50岁及以上的受试者的ω-3脂肪酸摄入与年龄相关的听力损失之间的关系。该研究发现,摄入更多的ω-3脂肪酸与入组时患年龄相关听力损失的可能性较低有关,并与在大约五年的随访中患年龄相关听觉损失的几率较低有关72。动物研究的结果还表明,ω-3脂肪酸酯可以预防听力损失的进展,改善耳蜗代谢,可能通过抑制炎症和同型半胱氨酸相关基因73。
6.肉碱:
肉碱在线粒体功能和细胞代谢中起着至关重要的作用。L肉碱及其衍生物乙酰L肉碱已在临床前研究中被检验其对年龄相关健康问题的潜在益处,包括阿尔茨海默病和帕金森病等神经系统疾病74,75。动物研究表明,补充L肉碱可以预防药物诱导的耳毒性76,和乙酰基L肉碱可以减少噪音对毛细胞的损伤77。然而,研究它们对年龄相关听力损失的可能预防作用,结果好坏参半78-80。
7.褪黑素:
褪黑激素是一种调节体内昼夜节律的激素,具有抗氧化和抗衰老特性。一项针对126名健康老年人的观察性研究发现,褪黑激素水平较低与听高音困难增加有关81。在动物研究中,发现补充褪黑激素可以减少与年龄相关82,83、噪音诱导84,85和耳毒性听力损失86,以及放疗引起的听力损失87。在一项实验室研究中,褪黑激素可以减少暴露于尼古丁的毛细胞中的氧化应激、炎症和异常蛋白质产生,这表明它可能在减少吸烟引起的听力损失方面发挥作用88。
